Post-Cardiac Rehabilitation Clothing India -- Dressing for Recovery and Exercise

Cardiac rehabilitation -- the structured programme of exercise, education, and support that follows open heart surgery or a major cardiac event -- begins when you are still healing. The sternum is still knitting together. The chest wound is still sensitive. And yet the physiotherapist is asking you to walk on a treadmill and do light arm exercises. What you wear to those sessions matters more than most people think, and getting dressed for cardiac rehab is a skill in itself.

What Is Cardiac Rehabilitation in India?

Cardiac rehabilitation is a medically supervised programme recommended after: open heart surgery (CABG or bypass), heart valve surgery, a heart attack, heart failure, or a coronary angioplasty. In India, cardiac rehab is available at most major cardiac centres in metro cities and is typically an outpatient programme that begins four to six weeks after surgery -- once the initial wound healing is underway.

A standard cardiac rehab programme in India typically involves:

  • Supervised walking and treadmill sessions
  • Light upper and lower body exercise
  • Breathing exercises and respiratory physiotherapy
  • Education sessions on diet, medication, and risk factor management
  • Psychological support components

The programme typically runs for six to twelve weeks, with sessions two to three times per week. Throughout this period, the sternum continues to heal, and any activity or clothing that puts stress on the chest wound needs to be managed carefully.

The Sternum Healing Timeline and Clothing

Open heart surgery requires the sternum (breastbone) to be divided to access the heart. It is wired back together at the end of surgery and heals over the following eight to twelve weeks. During this period:

  • The sternum is vulnerable to sternal instability if excessive force is applied through the arms or chest.
  • The chest wound -- a vertical midline incision -- is healing externally, and clothing that rubs against it can cause pain and delay healing.
  • Tight clothing across the chest restricts breathing depth, which interferes with the respiratory exercises that are part of cardiac rehab.
  • Bra straps and tight vest straps can press directly on the wound in women.

Standard cardiac surgeon advice for the first eight to twelve weeks after open heart surgery: no lifting more than 1 kg with either arm, no pushing or pulling with the arms, and no tight clothing across the chest. For detailed guidance on clothing during the immediate post-surgery period, see our guide to what to wear after open heart surgery.

What to Wear to Cardiac Rehab Sessions

During the First 8 Weeks of Rehab

In the early phase of cardiac rehab, the sternum is still not fully fused. The clothing priorities are:

  • No overhead lifting to dress: Use the tearaway side-opening t-shirt, which opens at the shoulder and down the side. This allows the garment to be placed on the body without lifting the arms overhead -- a movement that strains the healing sternum.
  • Loose across the chest: The garment must not press against the midline chest wound. Loose-fitting, soft cotton that drapes away from the chest is ideal.
  • Easy for the physio team to access the chest: At cardiac rehab, the physiotherapist will monitor breathing, check the wound periodically, and attach ECG leads. The velcro shoulder opening on the side-opening t-shirt allows the shoulder and upper chest to be exposed quickly without removing the garment.
  • Loose trousers with easy access: For the lower body, side-opening recovery pants or loose-waisted trousers that are easy to put on from a chair are ideal. This is not just comfort -- getting dressed without exertion is part of the early recovery protocol.

During Later Rehab and Transition Back to Regular Clothing

From around eight to twelve weeks post-surgery (depending on your surgeon's guidance and your healing rate), most cardiac patients can begin transitioning back to more regular clothing. The key milestones:

  • Once the sternal wound is fully healed externally, the risk of wound rubbing from clothing is resolved.
  • Once the sternum is confirmed stable at follow-up, the lifting restriction is progressively lifted, and overhead dressing becomes possible again.
  • Once the sternum is fully fused (typically twelve weeks, confirmed by your surgeon), there is no longer a structural reason to avoid overhead garments.

The transition back to regular clothing should be gradual. Start with loose t-shirts pulled over a now-healed chest before moving to fitted shirts. Women should reintroduce bras progressively -- starting with soft, wire-free wide-strap options before returning to regular bras, as the sternum area remains sensitive for several months even after fusion.

Layering for Temperature Regulation

Cardiac rehab facilities in India are often heavily air-conditioned. Many patients also experience temperature sensitivity during the recovery period (medication effects, circulation changes). Layering is important:

  • A base layer t-shirt (side-opening in early rehab) that can be worn alone for exercise.
  • A loose, front-opening cardigan or jacket for travelling to the facility and for rest periods between exercises.
  • Socks and enclosed, well-fitted exercise shoes (many cardiac patients have peripheral oedema in the ankles -- choose shoes that accommodate mild swelling).

For Women: The Bra Question

After open heart surgery, many women find that bra straps press directly on the chest wound and drain sites. In the first six to eight weeks, most women are more comfortable without a bra or with a very soft, wire-free crop-top style. At cardiac rehab, a loose-fitting side-opening t-shirt worn without a bra, or with a very soft sports bra whose straps do not cross the wound, is typically the most comfortable choice. Ask your cardiac physiotherapist for their specific recommendation.

Recovery Shorts for Physiotherapy

For lower body physiotherapy components -- leg exercises, cycling, walking assessments -- side-opening recovery shorts are useful. They give the physiotherapy team full access to the legs and ankles for assessment, and they can be removed and replaced easily if the session involves monitoring leads or ultrasound assessment of peripheral circulation.

Frequently Asked Questions

When can I start cardiac rehabilitation after open heart surgery in India?

Most cardiac rehab programmes in India begin at four to six weeks post-surgery, once the surgeon has confirmed adequate wound healing and sternal stability at the first major follow-up. Some supervised walking programmes begin earlier -- even in the hospital before discharge. Always follow your surgeon's specific clearance, not a general timeline.

Can I wear regular sportswear to cardiac rehab?

In later rehab (after eight to twelve weeks), yes -- once the sternum has healed sufficiently to allow overhead dressing and the chest wound is fully closed. In early rehab, regular sportswear that requires overhead lifting to put on, has tight chest bands, or has no quick-access opening is not recommended. Use adaptive clothing with front or side openings during the first eight weeks.

My legs are swollen from medication. How do I choose exercise shoes for rehab?

Many cardiac patients experience ankle oedema (swelling) from medications, particularly calcium channel blockers and vasodilators. Choose exercise shoes that are at least half a size larger than your normal size, with a wide toe box and easy fastening (velcro or slip-on rather than laces). Measure your feet for shoes in the late afternoon or evening when oedema is at its peak.

I am recovering from heart surgery and need to travel to cardiac rehab by auto or car. Any clothing tips?

Sitting in a vehicle seats the body in a way that pushes the seat belt directly across the sternum wound. Use a rolled towel or a small cushion between the seat belt and the wound to pad it. Wear the side-opening t-shirt to the session -- the shoulder velcro opening makes it easy to adjust the belt placement. Get in and out of the vehicle carefully, using your legs rather than pushing with your arms.

Cardiac rehabilitation is one of the most evidence-backed interventions after heart surgery, and completing the programme significantly improves long-term outcomes. The right clothing removes one barrier from that process -- making getting there easier, making the sessions more comfortable, and making the journey from surgical patient back to active person a little smoother.
